Name preview on 360 feedback pop up screen

Please can you add a name previous in the pop out when a person is asked to complete 360 feedback. Once they have opened the feedback response form they can no longer see who the feedback is for and we have had several incidents now of feedback being given about the wrong person. This is due to the fact we are in annual review cycle and people are being asked to give 360 feedback on multiple please.
